CAMP PARTICIPATION WAIVER
By agreeing to this form, I, as parent/guardian, permit Mount Saint Joseph Academy to use pictures of my child(ren) as a program participant in promotional literature, videos, MSJA website, and social media platforms. I understand my child(ren)’s name(s) will not be published.
Mount Saint Joseph Academy (MSJA) is not permitted to administer medication to program participants. In the event of a medical emergency, MSJA will administer first aid in the best interest of the child. Parents will be contacted if care is administered. Emergency allergy medications may be administered if provided by, and directed in writing by, the child’s parent/guardian.
Summer at the Mount programs are rooted in MSJA's mission of unity, inclusion, and respect for all. Campers are expected to reflect these values in their words, actions, and interactions with others each day.
Behavior that disrupts our sense of community, disrespects others, or fails to uphold a safe and welcoming environment will be viewed as inconsistent with Mount’s mission. Examples include, but are not limited to:
- Endangering the health or safety of other children and/or staff
- Bullying, teasing, or making fun of campers and/or staff
- Fighting or engaging in physical violence
- Using profanity, vulgarity, or obscene language or gestures
- Defacing or damaging property
Any camper whose behavior does not align with these expectations may have their participation limited or discontinued at the discretion of the Camp Leadership and Mount Saint Joseph Academy Administration.
Please note: If a camper is asked to leave camp for behavior that does not support MSJA’s mission or code of conduct, no refund will be issued.
I understand that my child's participation in this activity can expose her to dangers both from known and unanticipated risks. Acknowledging that such risks exist, I hereby release and discharge Mount Saint Joseph Academy, its agents, and employees from any and all claims or liability for personal injury or property damage she may suffer while participating in the activity; including, but not limited to, any claim arising out of any condition of the premises at which the activity is held or the conduct of any person in connection with the preparation for, supervision of, or conduct of any activity, whether planned or unplanned. I specifically agree to release and hereby release MSJA and the officers, agents, and employees for any negligence of the camp, or its officers, agents or employees.
I hereby give permission to the medical personnel selected by the camp counselor, coach, etc. to provide routine health care to my child, such as band-aids, minor wound treatment, etc. In the event of an emergency, if I cannot be reached, I hereby give permission to the physician selected by the camp to secure and administer treatment, including hospitalization, for the person named above. This completed release may be printed or shared for distribution.
